naviHealth’s decision support tools help identify patient needs, predict outcomes, and optimize post-acute care transitions. These proprietary solutions allow clinicians to initiate transitions earlier and patients to begin rehab faster, leading to reductions in length of stay and readmissions and improved functional gain.
