CarePort Guide is an interactive tool used by discharge planners, patients, families and care managers to find high-quality post-acute care for a patient’s specific needs. Through the CarePort-maintained, national database of post-acute providers, patients and families have greater visibility into available post-acute quality of care and services, which helps them make informed decisions about their care. Providers can also use CarePort Guide to fulfill regulatory requirements including the Conditions of Participation for discharge planning and the IMPACT Act by sharing quality information during the transition to the post-acute setting.

The interactive tool can be searched and filtered based on available post-acute provider information such as quality, preferred provider networks, geography and available clinical services. CarePort Guide also enables patients and families to review post-acute providers at the bedside, or through text message and email.
